By joining Signify Health’s nationwide clinical network, you’ll deliver meaningful moments in a person’s health care journey, helping close gaps in care and making time for meaningful conversations about peoples’ health.

As a nurse practitioner with our mission-driven team, you’ll visit people in their homes, identifying clinical and social needs and reconnecting health plan members with their primary care providers and health plans for follow-ups. You’ll conduct In-Home Health Evaluations, which include physical exams, preventive screenings, plus a medication and medical history review.
